Grant Opportunity:  CHSC will establish and support sustainable healthy communities as places where it is easier to practice healthy behaviors. Local-level organizations, supported by a statewide training and technical assistance center, will work within high-need communities to engage a variety of partners and decision-makers, and develop and execute a comprehensive implementation plan.

The New York State Department of Health anticipates awarding up to 25 grants to organizations throughout the state. 

Eligibility: 

• School districts
• Boards of Cooperative Educational Services (BOCES) agencies
• 501(c)(3) organizations
• Tribal organizations
• Early care and education organizations including Child Care Resource and Referral (CCR&R) agencies
• Academic institutions in New York State
• Municipal agencies in New York State

Grant Amount: $305,000

Grant Period:  5 years (2020-2025)

Application Deadline: Tuesday, February 4, 2020
