Creating Family-to-Family Health Centers for Families of Children with Special Health Needs

Grant Opportunity: The mission of Health Resources & Services Administration's (HRSA) Family-to-Family Health Information Centers grant is to promote the health and well-being of children and youth with special health care needs (CYSHCN) and their families by developing centers to provide information, education, technical assistance, and peer support. Through the creation of Family-to-Family Health Centers, HRSA aims to promote family engagement in their child's healthcare in order to advance a system of services in which families and health professionals partner in health care decision-making.
 
Eligibility: Public and private entities located in the United States and American territories are eligible to apply. Domestic faith-based and community-based organizations, tribes, and tribal organizations are also eligible to apply.
 
Grant Amount and Grant Period: A maximum of $96,750 per award will be funded over a five year grant period between June 1st, 2022 through May 31st, 2027.
 
Deadline: January 5th, 2022
